An online web based surveillance to gather traveller information pertaining to the purpose of improving public health. This is in accordance with Public Health Act 2009, thus false information is an offence.
Each passenger is required to complete an individual form. Children traveling with a guardian/parent, will also need to complete their own form.

- Your travel details, including times and dates
Upon Submission of the form
Travelers will receive a confirmation notificationThe notification will contain their Unique Health Code (UHC) which they will present upon arriving in Tanzania at health desk
